Mango ACO Care Coordination Alerts

Mango ACO, LLC ("Mango ACO," "we," "us") operates the Mango ACO Care Coordination Alerts program, a text messaging service that sends transactional care coordination notifications to participating healthcare providers. Messages notify enrolled providers that a patient attributed to them has been discharged from a hospital or emergency department and direct the provider to a secure portal for details. Messages do not contain patient health information. This program sends transactional notifications only; it does not send marketing or promotional messages.
Enrollment is limited to licensed healthcare providers participating in Mango ACO. Providers enroll by initialing the SMS consent block on the Mango ACO Provider SMS Consent Form during onboarding. Consent to receive text messages is optional and is not a condition of participation in Mango ACO.
Message frequency varies and depends on patient care events. Providers typically receive several messages per month, but volume may be higher or lower in any given period.
Mango ACO does not charge for this service. Message and data rates may apply according to your mobile carrier plan. Contact your carrier for details about your plan.
You may opt out at any time by replying STOP to any message. After texting STOP, you will receive a confirmation from the carrier network and will not receive further messages unless you re-enroll by texting START or UNSTOP. You may also contact us using the information in Section 8 to withdraw consent.

Mobile carriers are not liable for delayed or undelivered messages. Message delivery is subject to effective transmission by your mobile carrier and is outside our control.
